Cosmetic breast reduction surgeries\u2014those that were not considered medically necessary and paid for out of pocket\u2014increased by 54%, from 2019 to 2022, according to the<a href=\"https:\/\/www.plasticsurgery.org\/documents\/news\/Statistics\/2022\/plastic-surgery-statistics-report-2022.pdf\" target=\"_blank\" aria-label=\"Go to https:\/\/www.plasticsurgery.org\/documents\/news\/Statistics\/2022\/plastic-surgery-statistics-report-2022.pdf\" rel=\"noopener\" class=\"sc-80b85506-0 ovBKL\"> American Society of Plastic Surgeons (ASPS)<\/a>.<\/p>\n<p>ASPS suggests several factors may have contributed to this trend, including an increased health and fitness focus, remote work and flexible schedules allowing for easier surgery recovery, and more time for reflection about personal comfort and well-being.<\/p>\n<p><a href=\"https:\/\/www.mayoclinic.org\/tests-procedures\/breast-reduction-surgery\/about\/pac-20385246\" target=\"_blank\" aria-label=\"Go to https:\/\/www.mayoclinic.org\/tests-procedures\/breast-reduction-surgery\/about\/pac-20385246\" rel=\"noopener\" class=\"sc-80b85506-0 ovBKL\">Benefits of breast reduction surgery<\/a> may include relief from upper back, neck, and shoulder pain, an increased ability to exercise and participate in physical activities, and an improved self-image.<a href=\"https:\/\/www.mayoclinic.org\/tests-procedures\/breast-reduction-surgery\/about\/pac-20385246\" target=\"_blank\" aria-label=\"Go to https:\/\/www.mayoclinic.org\/tests-procedures\/breast-reduction-surgery\/about\/pac-20385246\" rel=\"noopener\" class=\"sc-80b85506-0 ovBKL\">\u00a0<\/a><\/p>\n<h2 class=\"wp-block-heading\"><strong>Older women and breast reduction surgery<\/strong><\/h2>\n<p>Dr. Lynn Damitz, chief of the<a href=\"https:\/\/www.med.unc.edu\/surgery\/plastic\/\" target=\"_blank\" aria-label=\"Go to https:\/\/www.med.unc.edu\/surgery\/plastic\/\" rel=\"noopener\" class=\"sc-80b85506-0 ovBKL\"> Division of Plastic and Reconstructive Surgery<\/a> and Director of Aesthetic Surgery at the UNC School of Medicine in Chapel Hill, NC, and ASPS board vice president of health policy and advocacy, has seen an increasing number of women 65 and over pursuing breast reduction surgery.<\/p>\n<p>\u201cI\u2019ve definitely seen an increase in the last two decades,\u201d she says. \u201cMy patients over 65 are highly motivated. Often, they have been thinking about doing the surgery for a long time, and are ready to do something for themselves.\u201d<\/p>\n<h2 class=\"wp-block-heading\"><strong>The cost of breast reduction surgery<\/strong><\/h2>\n<p>The cost of the surgery can be significant. According to ASPS, the<a href=\"https:\/\/www.plasticsurgery.org\/reconstructive-procedures\/breast-reduction\/costs\" target=\"_blank\" aria-label=\"Go to https:\/\/www.plasticsurgery.org\/reconstructive-procedures\/breast-reduction\/costs\" rel=\"noopener\" class=\"sc-80b85506-0 ovBKL\"> average cost<\/a> of a surgeon\u2019s fee for cosmetic breast reduction is $6,771. Additional fees may include anesthesia and hospital or surgical facility costs, medical tests, prescriptions, and post-surgery garments.<\/p>\n<p>\u201cWhile many women choose to pay for the procedure out of pocket, commercial insurance, as well as Medicare and <a href=\"https:\/\/fortune.com\/well\/2023\/08\/09\/pros-cons-of-medicare-advantage-plans\/\" target=\"_self\" aria-label=\"Go to https:\/\/fortune.com\/well\/2023\/08\/09\/pros-cons-of-medicare-advantage-plans\/\" data-type=\"link\" data-id=\"https:\/\/fortune.com\/well\/2023\/08\/09\/pros-cons-of-medicare-advantage-plans\/\" class=\"sc-80b85506-0 ovBKL\" rel=\"noopener\">Medicare Advantage<\/a>, may cover it,\u201d says Damitz.<\/p>\n<h2 class=\"wp-block-heading\"><strong>How to qualify for Medicare and Medicare Advantage coverage of breast reduction surgery<\/strong><\/h2>\n<p>To get coverage for breast reduction surgery by Medicare or <a href=\"https:\/\/fortune.com\/well\/article\/medicare-advantage-plans-will-get-skimpier-and-costlier\/\" target=\"_self\" aria-label=\"Go to https:\/\/fortune.com\/well\/article\/medicare-advantage-plans-will-get-skimpier-and-costlier\/\" class=\"sc-80b85506-0 ovBKL\" rel=\"noopener\">Medicare Advantage<\/a>, it needs to be<a href=\"https:\/\/www.cms.gov\/medicare-coverage-database\/view\/lcd.aspx?lcdid=35001\" target=\"_blank\" aria-label=\"Go to https:\/\/www.cms.gov\/medicare-coverage-database\/view\/lcd.aspx?lcdid=35001\" rel=\"noopener\" class=\"sc-80b85506-0 ovBKL\"> medically necessary, not cosmetic<\/a>. Specifically, the surgery should be necessary:<\/p>\n<ul>\n<li>To reduce decrease or eliminate symptoms caused by breast size<\/li>\n<li>To reduce the size of a breast to make it symmetrical with a breast reconstructed after breast cancer surgery<\/li>\n<\/ul>\n<p>Large breasts may cause a number of uncomfortable symptoms, including back pain, neck pain, heaviness, shoulder grooving, posture problems, neuropathic pain, numbness, discomfort with skin-on-skin contact, rashes, skin conditions, and hygiene issues.<\/p>\n<p>\u201cBefore Medicare or Medicare Advantage coverage is considered, patients must have experienced a number of these symptoms,\u201d says Damitz. In addition, the<a href=\"https:\/\/www.cms.gov\/medicare-coverage-database\/view\/lcd.aspx?lcdid=39051&amp;ver=7&amp;keyword=mammoplasty&amp;keywordType=starts&amp;areaId=all&amp;docType=F&amp;contractOption=all&amp;sortBy=relevance&amp;bc=1\" target=\"_blank\" aria-label=\"Go to https:\/\/www.cms.gov\/medicare-coverage-database\/view\/lcd.aspx?lcdid=39051&amp;ver=7&amp;keyword=mammoplasty&amp;keywordType=starts&amp;areaId=all&amp;docType=F&amp;contractOption=all&amp;sortBy=relevance&amp;bc=1\" rel=\"noopener\" class=\"sc-80b85506-0 ovBKL\"> Centers for Medicare &amp; Medicaid Services (CMS)<\/a> details these rules:<\/p>\n<ul>\n<li>Surgeons must make sure that patients have tried<a href=\"https:\/\/www.cms.gov\/medicare-coverage-database\/view\/lcd.aspx?lcdid=39051&amp;ver=7&amp;keyword=mammoplasty&amp;keywordType=starts&amp;areaId=all&amp;docType=F&amp;contractOption=all&amp;sortBy=relevance&amp;bc=1\" target=\"_blank\" aria-label=\"Go to https:\/\/www.cms.gov\/medicare-coverage-database\/view\/lcd.aspx?lcdid=39051&amp;ver=7&amp;keyword=mammoplasty&amp;keywordType=starts&amp;areaId=all&amp;docType=F&amp;contractOption=all&amp;sortBy=relevance&amp;bc=1\" rel=\"noopener\" class=\"sc-80b85506-0 ovBKL\"> non-surgical interventions<\/a> to alleviate breast pain or chafing, such as chiropractic care, physical therapy, and dermatologic treatments<\/li>\n<li>The symptom(s) must have been present for a minimum of six months<\/li>\n<li>Interventions and medical treatment were not able to alleviate symptoms adequately<\/li>\n<li>The patient has been notified of the risks of complications<\/li>\n<li>The notes indicate the proposed amount of tissue to be removed and the rationale supporting that determination<\/li>\n<\/ul>\n<p>In addition, CMS rules state that the amount of breast tissue to be removed must be proportional to the body surface area, based on a measurement called the<a href=\"https:\/\/www.cms.gov\/medicare-coverage-database\/view\/lcd.aspx?lcdid=39051&amp;ver=7&amp;keyword=mammoplasty&amp;keywordType=starts&amp;areaId=all&amp;docType=F&amp;contractOption=all&amp;sortBy=relevance&amp;bc=1\" target=\"_blank\" aria-label=\"Go to https:\/\/www.cms.gov\/medicare-coverage-database\/view\/lcd.aspx?lcdid=39051&amp;ver=7&amp;keyword=mammoplasty&amp;keywordType=starts&amp;areaId=all&amp;docType=F&amp;contractOption=all&amp;sortBy=relevance&amp;bc=1\" rel=\"noopener\" class=\"sc-80b85506-0 ovBKL\"> Schnur scale<\/a>. Specifically, \u201cIf the individual\u2019s body surface area and weight of breast tissue removed fall above the 22nd percentile, then the surgery is considered medically reasonable and necessary with the appropriate criteria. If only one breast meets the Schnur scale criteria; breast tissue may be removed from the other breast to achieve symmetry.\u201d<\/p>\n<p>\u201cThis rule considers how disproportionate one\u2019s breasts are compared to the rest of the body,\u201d says Damitz. If you are overweight, you may not meet the Schnur scale criteria, says Damitz.<\/p>\n<h2 class=\"wp-block-heading\"><strong>Medicare and Medicare Advantage coverage of breast reduction surgery<\/strong><\/h2>\n<p>Only some surgeons accept insurance coverage for breast reduction surgery. Damitz is one of them, and a significant portion of her breast reduction surgeries are covered by insurance. Of the breast reductions she performs, 29% are covered by Medicare and Medicare Advantage, she says.<\/p>\n<p>The approval process may be different for Medicare or Medicare Advantage plan members.<\/p>\n<p>\u201cMedicare doesn\u2019t offer prior approval of breast reduction surgery, but some Medicare Advantage plans do,\u201d says Damitz. \u201cThat means that even if you think you meet the qualifications for Medicare coverage, you may choose to pay upfront for the surgery, and hope for Medicare reimbursement afterward,\u201d she says. It\u2019s important to note that if Medicare does not reimburse afterward, however, these fees may be substantially higher than a prenegotiated upfront fee, says Damitz.<\/p>\n<p>If the patient\u2019s Medicare Advantage plan offers prior approval, it may pay upfront for the surgery fees.<\/p>\n<p>\u201cEvery Medicare Advantage plan criteria is different,\u201d says Damitz. \u201cIt\u2019s helpful for patients to contact their insurers ahead of time and ask what the criteria are.
