Perhaps you don\u2019t have as much time to hit the gym as you\u2019d like, or have a medical condition that prevents you from exercising at all. Maybe you hate working out. Wouldn\u2019t it be easier if you could just take a pill to reap the rewards of exercise?<\/p>\n<div>\n<p>The idea may not be so far-fetched. After testing new drug compounds that appear to mimic the physical benefits of exercise in rodents, scientists say a pill may someday be able to do the same in humans. <a href=\"https:\/\/profiles.wustl.edu\/en\/persons\/bahaa-eldien-elgendy\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">Bahaa Elgendy, Ph.D.<\/a>, a medicinal chemist and associate professor of anesthesiology at the <a href=\"https:\/\/medicine.wustl.edu\/\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">Washington University School of Medicine in St. Louis<\/a>, was scheduled to present his team\u2019s findings Monday at the spring meeting of the <a href=\"https:\/\/www.acs.org\/\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">American Chemical Society (ACS)<\/a>.<\/p>\n<p>As you age, you gradually lose muscle mass, strength, and function\u2014a type of atrophy called <a href=\"https:\/\/my.clevelandclinic.org\/health\/diseases\/23167-sarcopenia\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">sarcopenia<\/a>. Elgendy\u2019s team hopes an exercise-mimicking pill could treat muscle atrophy and improve the physical fitness of people with serious ailments such as cancer, heart failure, and <a href=\"https:\/\/fortune.com\/well\/article\/abdominal-fat-alzheimers-disease-men-women\/\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">neurodegenerative diseases<\/a>, according to an <a href=\"https:\/\/www.acs.org\/pressroom\/presspacs\/2024\/march\/mimicking-exercise-with-a-pill.html\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">ACS news release<\/a>.<\/p>\n<p>\u201cWe\u2019re not saying by any means or forms that people shouldn\u2019t exercise,\u201d Elgendy tells <em>Fortune<\/em> ahead of his presentation. \u201cBut [the drug] hopefully will help people who cannot exercise. And, in other cases, it can complement exercise programs to give more benefits to patients as well.<\/p>\n<p>\u201cOr, it can be combined with the new wave of drugs: anti-diabetic drugs and <a href=\"https:\/\/fortune.com\/well\/2024\/03\/01\/ozempic-weight-loss-drugs-not-solution-obesity-who-world-health\/\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">drugs that are used for obesity and weight loss<\/a>.\u201d<\/p>\n<p>Elgendy and his team spent about a decade developing a compound called <a href=\"https:\/\/jpet.aspetjournals.org\/content\/388\/2\/232.long\" target=\"_blank\" rel=\"noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">SLU-PP-332<\/a>. It activates a group of proteins called <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC7084735\/\" target=\"_blank\" rel=\"noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">estrogen-related receptors<\/a>\u2014ERR\u03b1, ERR\u03b2, and ERR\u03b3\u2014which Elgendy says are \u201cresponsible for activating some of the most important metabolic pathways in tissues with high-energy demand.\u201d The most elusive target, ERR\u03b1, regulates exercise-induced stress adaptation as well as physiological muscle processes.\u00a0Mice treated with SLU-PP-332 showed improved endurance running on a rodent treadmill. It also increased a fatigue-resistant muscle fiber in the animals, researchers found.<\/p>\n<p>Next, the team created new, patentable molecules with the goal of making them more safe, potent, and efficacious than SLU-PP-332, Elgendy says. Further study of rat heart cells suggests the new compounds do more strongly mimic the effects of exercise.<\/p>\n<p>\u201cThe new generations we developed that I\u2019m going to talk about today, these are predicted to make it hopefully to the clinic one day in the next five years,\u201d Elgendy tells <em>Fortune<\/em>. \u201cThe translation from animals to humans takes a long time. We need to do a lot more preclinical testing, which is vital to ensure safety.\u201d<\/p>\n<figure class=\"wp-block-embed is-type-video is-provider-youtube wp-block-embed-youtube wp-embed-aspect-16-9 wp-has-aspect-ratio\">\n<p><iframe title=\"How a simple pill can mimic the effects of exercise\" width=\"1200\" height=\"675\" src=\"https:\/\/www.youtube.com\/embed\/IIcUrSIEa-4?feature=oembed\" frameborder=\"0\" allow=\"accelerometer; autoplay; clipboard-write; encrypted-media; gyroscope; picture-in-picture; web-share\" allowfullscreen><\/iframe><\/p>\n<\/figure>\n<h2 class=\"wp-block-heading\">What are exercise mimetics?<\/h2>\n<p>Exercise mimetics are a proposed class of drugs that aim to do exactly what the name suggests: mimic the health benefits of exercise. SLU-PP-332 and the new compounds fall into this category.<\/p>\n<p>\u201cThese compounds simulate some of the same adaptations that occur in muscle with exercise. So, your muscles think that they are exercising even though they may not be,\u201d <a href=\"https:\/\/pharmacy.ufl.edu\/profile\/burris-thomas\/\" target=\"_blank\" rel=\"noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">Thomas Burris, Ph.D.<\/a>, chair of the <a href=\"https:\/\/pd.pharmacy.ufl.edu\/\" target=\"_blank\" rel=\"noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">Department of Pharmacodynamics at the University of Florida<\/a>, tells <em>Fortune<\/em>. \u201cThey improve metabolic health, they cause weight loss, fat mass loss, improved insulin sensitivity, [and] improved exercise endurance.\u201d<\/p>\n<p>Burris is part of Elgendy\u2019s research team. With the skyrocketing popularity of <a href=\"https:\/\/www.mayoclinic.org\/diseases-conditions\/type-2-diabetes\/expert-answers\/byetta\/faq-20057955\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">GLP-1 agonists<\/a>\u2014the class of drugs that treat obesity such as Wegovy and Zepbound\u2014people are increasingly turning to pharmaceuticals to aid in weight loss, Burris says. But he stresses that GLP-1s cause the loss of muscle mass and function in addition to body fat. The exercise mimetics he and his colleagues are studying may temper those effects.<\/p>\n<p>\u201cThere\u2019s a great need for something to be used in combination so that people who are losing that weight are primarily losing fat mass and retaining good-quality muscle, because that can lead to future complications,\u201d Burris tells <em>Fortune<\/em>. \u201cOlder people who go on [GLP-1s] who already have maybe some reduced muscle quality, losing that mass could lead to some disabilities.\u201d<\/p>\n<p><a href=\"https:\/\/phmtox.msu.edu\/people\/alanjami\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">Jamie Alan, Pharm.D.<\/a>, an associate professor in the <a href=\"https:\/\/phmtox.msu.edu\/\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">Department of Pharmacology and Toxicology at Michigan State University<\/a>, wasn\u2019t involved in the research but tells <em>Fortune<\/em> the results are exciting. She\u2019s intrigued by the team\u2019s plan to focus on the brain in future study.<\/p>\n<p>\u201cThese particular drugs that they used in this first go-around, they don\u2019t cross into the brain at all,\u201d Alan says, referring to SLU-PP-332. \u201c[Researchers] said it could potentially have more far-reaching benefits if we design a drug that still hits these receptors but has the ability to get into the brain.\u201d That way, they can study the compounds as possible treatments for neurodegenerative diseases.
