Employee efficiency and productivity could plummet, too, not to mention the corrosion of company culture. For these reasons, billionaire entrepreneur and Cost Plus Drugs cofounder <a href=\"https:\/\/fortune.com\/tag\/mark-cuban\/\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">Mark Cuban<\/a> urges business leaders to take a hard look at how their health dollars are spent.<\/p>\n<div>\n<p>\u201cEven though health insurance costs are the second-largest line item after payroll, CEOs don\u2019t really understand it or know anything about it,\u201d Cuban tells <em>Fortune<\/em>. \u201cYou have employees that are more worried about whether their kids can get their medications paid for by the company insurance, or whether they\u2019re going to be able to get the surgery that they know they need and that the doctor recommends, but the insurance company is not approving.\u201d<\/p>\n<p>Cuban spoke with <em>Fortune<\/em> on Monday after participating in <a href=\"https:\/\/www.youtube.com\/watch?v=DiA6hAslOFg\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">a White House roundtable discussion<\/a> on lowering health care costs and bringing transparency to the role of prescription drug middlemen. He was joined by <a href=\"https:\/\/www.linkedin.com\/in\/alex-oshmyansky-md-phd-06a3174b\/\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">Dr. Alex Oshmyansky<\/a>, the CEO and cofounder of the <a href=\"https:\/\/costplusdrugs.com\/\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">Mark Cuban Cost Plus Drug Company<\/a>, a low-cost drug supplier. Before government leaders including Secretary of Health and Human Services <a href=\"https:\/\/fortune.com\/2023\/04\/09\/abortion-pill-ruling-reckless-and-not-america-says-health-secretary-xavier-becerra\/\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">Xavier Becerra<\/a> and Federal Trade Commission chair <a href=\"https:\/\/fortune.com\/2024\/02\/26\/ftc-chair-lina-khan-house-judiciary-committee-report-classic-mismanagement\/\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">Lina Khan<\/a>, Cuban and Oshmyansky accused those middlemen companies, known as pharmacy benefit managers, or PBMs, of prioritizing profits over patients. <\/p>\n<p>These third parties act as intermediaries between health insurance providers and drug manufacturers. Cost Plus Drugs launched its online marketplace in early 2022, with the mission to buy drugs and sell them directly, and thereby more cheaply, to consumers. Oshmyansky announced at the White House that the Dallas-based company would <a href=\"https:\/\/fortune.com\/well\/article\/mark-cuban-cost-plus-drugs-dallas-pharmaceutical-plant\/\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">begin manufacturing its own medications<\/a> this week.<\/p>\n<h2 class=\"wp-block-heading\">PBMs respond to Cuban\u2019s cost claims<\/h2>\n<p>The so-called \u201cbig three PBMs\u201d\u2014CVS Caremark, Cigna\u2019s Express Scripts, and UnitedHealth\u2019s Optum Rx\u2014controlled 80% of the market in 2022, according to the <a href=\"https:\/\/www.drugchannels.net\/2023\/05\/the-top-pharmacy-benefit-managers-of.html\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">Drug Channels Institute<\/a>, and faced particular ire during the roundtable.\u00a0<\/p>\n<p>\u201cIf you\u2019re using a big three PBM, you are getting ripped off,\u201d Cuban tells <em>Fortune<\/em>. \u201cPeriod. End of story.\u201d<\/p>\n<p>CVS Caremark spokesperson Phil Blando tells <em>Fortune<\/em> in an email statement: \u201cEmployers, unions, health plans, and government programs work with CVS Caremark precisely because we deliver for them\u2014lower drug costs, better health outcomes, and broad pharmacy access. Through our True Cost, CostVantage and Choice Formulary initiatives, we are the leading agent of change, innovation, and transparency in the market.\u201d<\/p>\n<p>An Express Scripts representative tells <em>Fortune<\/em> via email: \u201cOur clients choose to work with us because we offer broad flexibility and choice, the clinical expertise to ensure better health outcomes for the people they serve, and the negotiating muscle to secure savings across the thousands of brand, specialty, and generic medications available today, not just a handful of generic medications.\u201d<\/p>\n<p>Optum Rx didn\u2019t respond by deadline.<\/p>\n<p>The <a href=\"https:\/\/www.pcmanet.org\/\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">Pharmaceutical Care Management Association (PCMA)<\/a>, a trade organization representing PBMs, places blame on drug manufacturers. The <a href=\"https:\/\/www.pcmanet.org\/transparency\/\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \"><em>Transparency<\/em> section of the PCMA website<\/a> contends there are right and wrong ways to be transparent about prescription prices. \u201cSpecific price concessions that allow drugmakers to realize they can discount less\u201d is one such move PCMA says increases costs.<\/p>\n<p><a href=\"https:\/\/fortune.com\/well\/2023\/12\/21\/march-in-rights-biden-lower-excessive-drug-prices\/\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">Neera Tanden<\/a>, domestic policy advisor to <a href=\"https:\/\/fortune.com\/tag\/joe-biden\/\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">President Joe Biden<\/a>, said during Monday\u2019s roundtable that PBMs play a critical role in negotiating lower drug prices.<\/p>\n<p>\u201cWe have to remember that the original conception of PBMs is that they had the power to negotiate against Big Pharma,\u201d Tanden said. \u201cHowever, we have seen in current practices that that is a real challenge.\u201d<\/p>\n<p>The quest for PBM reform has bipartisan support in Congress. At the same time, pharmacy groups have criticized politicians for their lack of consensus and failure to pass what they view as meaningful change into law. As Cost Plus Drugs got off the ground, he says he compared the prices of generic prescription medications he\u2019d been paying for to those offered by his new pharmacy.<\/p>\n<p>\u201cWhat we spent $160,000 on at the [<a href=\"https:\/\/fortune.com\/tag\/nba\/\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">Dallas Mavericks<\/a>], we could have purchased for $19,000 for Cost Plus,\u201d he tells <em>Fortune<\/em>. \u201cWe decided once that plan ended last year, we were going to replace it, and so I wanted to go through the entire process of understanding what I did right or wrong.\u201d<\/p>\n<p>He first turned to the employee benefits consultant who had assured him he was getting a great deal\u2014someone whose advice also cost $30 per employee per month.<\/p>\n<p>\u201cThat\u2019s insane, that was millions of dollars,\u201d Cuban says. \u201cThe person who put me into a program where I was paying eight times more than I should have for generic medication, they\u2019re done.\u201d<\/p>\n<p>Around the time he dropped the consultant, Cuban was planning to get a CT colonoscopy, which he says would\u2019ve cost more than $2,000 through insurance. Yet, the walk-in cost for the same procedure was less than $500.<\/p>\n<p>\u201cWhat is the purpose of this insurance company that I\u2019m working with\u2014and the PBM that they\u2019re connecting me to\u2014when I can just walk in off the street and save myself a ton of money?\u201d Cuban asks. \u201cWhat we\u2019ve done at my companies is we\u2019ve walked away from the traditional way.\u201d<\/p>\n<p>Cuban decided his companies wouldn\u2019t work with PBMs that deal in rebates, including the big three. A study of rebates for nearly 450 branded drugs, published in 2021 in the journal <a href=\"https:\/\/jamanetwork.com\/journals\/jamanetworkopen\/fullarticle\/2780950\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \"><em>JAMA Network Open<\/em><\/a>, found an association between increased rebate sizes and increased out-of-pocket costs for people with <a href=\"https:\/\/fortune.com\/tag\/medicare\/\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">Medicare<\/a>, commercial insurance, or no insurance. The PBM wouldn\u2019t allow the use of <a href=\"https:\/\/www.fda.gov\/drugs\/therapeutic-biologics-applications-bla\/biosimilars\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">biosimilars<\/a>, lower-cost biologic medications. Today, Cost Plus Drugs sells the biosimilar <a href=\"https:\/\/costplusdrugs.com\/medications\/yusimry-40mg-0_8ml-box-of-2-pen-injectors\/\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">Yusimry (adalimumab-aqvh)<\/a> for 41% off the retail price.<\/p>\n<p>For his employees, Cuban now uses <a href=\"https:\/\/affirmedrx.com\/\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">AffirmedRx<\/a>, a <a href=\"https:\/\/ncpa.org\/spread-pricing-101\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">pass-through PBM<\/a>, meaning it\u2019s designed to collect revenue from administrative fees instead of discounts and rebates, and pass savings along to customers.\u00a0<\/p>\n<p>\u201cEverything, we just pay for on a transactional basis and they do what we tell them to do,\u201d Cuban tells <em>Fortune<\/em>. \u201cThat was an easy switch.\u201d<\/p>\n<p>CEOs shouldn\u2019t depend solely on their HR departments and CFOs to untangle such costs, Cuban advises, stressing that the oldest and sickest employees end up bearing the brunt of copays and high deductibles when employers make uninformed health coverage decisions.<\/p>\n<p>\u201cI really think that we\u2019re going to start seeing a trend for health care CFOs, in any company with more than 1,000 employees, where there\u2019s somebody focused on the economics and <a href=\"https:\/\/fortune.com\/well\/2024\/03\/06\/kpmg-launches-well-being-initiative-workers-risk-burnout\/\" target=\"_blank\" rel=\"noreferrer noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">wellness of their employees<\/a>,\u201d Cuban says.<\/p>\n<p><em><a href=\"https:\/\/fortune.com\/company\/cvs-health\/\" target=\"_blank\" rel=\"noopener\" class=\"sc-47dba8f0-0 iRbseu styledLinkColor \">CVS Health<\/a> is a sponsor of Fortune WELL.<\/em><\/p>\n<p><strong>For more on the Mark Cuban Cost Plus Drug Company:<\/strong><\/p>\n<\/div>\n<p>[ad_2]<br \/>\n<br \/><a href=\"https:\/\/fortune.com\/well\/2024\/03\/08\/mark-cuban-ceo-health-care-costs\/\">Source link <\/a><\/p>\n","protected":false},"excerpt":{"rendered":"<p>[ad_1] When a CEO fails to grasp the nuances of their company\u2019s health care costs, it\u2019s not just the bottom line that suffers.
